– Drug class: Mast cell stabilizer
– Indication: For the management of mastocytosis, also used off-label for eosinophilic esophagitis
Dosing: Adults: 200mg 3-4 times daily before meals and at bedtime; may increase to 400mg 4 times daily. Children: 20mg/kg/day divided 3-4 times daily.
– Administration: Taken by mouth, best absorbed when held under tongue briefly before swallowing. Should not be mixed with food/liquids.
– MOA: Inhibits mast cell degranulation by blocking calcium ion influx. Prevents release of inflammatory mediators like histamine.
– Onset: Symptom relief may take up to 4 weeks. Full benefit may take up to 8 weeks.
– Duration: Effects last about 3-6 hours after each dose.
– Side Effects: Generally well tolerated. Possible mild GI upset, headache, nasal congestion, bad taste.
– Contraindications: Hypersensitivity to cromolyn or any ingredients.
– Interactions: No major drug-drug interactions. Can be used concomitantly with antihistamines.
– Considerations: Not for treatment of acute mast cell mediator release or anaphylaxis. Use caution in renal impairment.
